Physical Therapy is a health care profession that treats individuals of all ages who have medical problems or other health related conditions that limit their abilities to move and perform functional activities in their daily lives. The physical therapist examines each individual and develops a plan using treatment techniques to promote the ability to move, reduce pain, restore function and prevent disability. In addition, Physical Therapists work with individuals to prevent the loss of mobility before it occurs by developing fitness and wellness oriented programs for healthier and more active lifestyles.
We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physician’s referral for therapy to be covered. We also offer treatments, services and/or products that do NOT require a referral and are available on a self-pay basis.
Most insurance companies pay for physical therapy. However, amounts paid and number of visits allowed are highly variable and depend on the policies of your carrier. Please contact your insurance company or one of our centers for assistance with insurance questions.
How much will I owe for each session?
Many insurance companies require a co-pay that you are responsible for paying at the time of service. We accept cash, check, debit, and major credit cards for a co-pay or service not covered by your insurance plan at one of our centers.
Please check with your insurance company or insurance card to determine your co-pay. You may also have a deductible and a percentage allocation amount required by your insurance company. Again, please refer to your policy to determine the amount you may owe or contact one of our centers so we may assist you.
As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.
Please dress comfortably for your appointments. You will be required to complete the new patient paperwork, which can be filled out prior to your initial appointment. You should arrive approximately 10 minutes early to be ready for your appointment time with your therapist.
After your consultation, you can also expect a thorough, interactive evaluation from a Licensed Therapist in order to establish a customized treatment plan that will assist you in meeting your functional goals. During this evaluation you will have the opportunity to discuss your condition, help set goals for your therapy, and gain a good understanding of what you may expect from attending physical therapy visits including prognosis, treatment interventions and approximately how long it should take. At the end of the first treatment, you should receive a copy of your home exercises followed by a quick check out process. Initial visits typically last about 90 minutes.
On all follow up visits, you simply sign in at the desk in the lobby and expect to wait no longer than 5 minutes for a qualified therapist to assist you to the treatment area. From here you will continue your treatment progression. Follow up visits typically last 30-60 minutes.

“Had a wonderful experience here as I’ve been dealing with my back issues. Dr. Small is very professional and listens/understands as all PTs should, but many don’t. I left with lots of strategies and ways I can self-treat my issues with stretching–overall, though, he’s got a great way about him and the support staff was super friendly and accommodating. If you’re having issues, it’s highly recommended.”
“Evan Blackburn was awesome. At first I was a lazy bum and feeling super down after my surgery, but Evan whipped me into shape. Awesome place, but hope I don’t ever have to go back again! LOL!”
“VERY technically competent. Listens to the patient, adapts regime to help w/o over-stressing patient’s injury. Monty Strickland and all the staff are also very friendly.”
“Qualified and conscientious staff. Friendly too and show interest and concern for patients.”
“Awesome care and customer service. Getting you better and back in the “game” is their top priority. Lance listens and discusses treatment/rehab options to get you back where you need to be.”
We believe in recruiting, developing and retaining the best talent in the industry. One of our strategic objectives is to raise the standard of care and leadership in physical therapy and sports rehabilitation.

The program is designed to select a highly qualified and diverse intern team that brings together valuable differences and fresh perspectives. Candidates for participation in the AFPTC internship program will generally fit into four categories, which are specific to the degree of education and expertise that the intern brings to the position. These four categories also determine the onboarding criteria that must be met in order for the intern to be in compliance with AFPTC Internship guidelines.
Our internships are designed to take advantage of each person’s specialized skills, making every effort to give the intern an opportunity to work on meaningful projects and participate in staff activities whenever possible and appropriate.
